Objectives: the aim of this study was to identify the white spot lesions in orthodontically treated patients through their bacterial microbiota and international caries detection and assessment system (ICDAS), and correlate the existence of white spot lesions with the oral hygiene status of the patients. Materials and methods: The study included 20 White spot lesions-free and 20 white spot lesions positive subjects with age range (12-16 years). The study included 19 females and 1 male in the White spot lesions-free group; and 15 females and 5 males in the white spot lesions positive group. The subjects were recruited from outpatient clinic of Orthodontic Department, Cairo University. Saliva samples were collected from the subjects by collection of saliva using plastic disposable syringe with written code of each saliva sample on tape then transferred to Microbiology Department's lab in the same day. Culture-based method was used to evaluate the count of cariogenic microbial pathogens (Non-S. Mutans Streptococci and Actinomyces). Results: The results showed statistical significant difference between white spot lesions-free group (group A) and white spot lesions positive group (group B). Group B had statistically significant increase in the count of Non Mutans Streptococci and Actinomyces bacteria, and the plaque index score than white spot lesions free group
