This study included 96 patients who were diagnosed with uterine sarcoma and presented to National cancer institute from January 2010 to January 2018 and they represent 16% of all Uterine malignancies presented to National cancer institute in this time interval, So they represent large number of patients who are worthy of medical and research concern. There is decreased locoregional recurrence in patients who did pelvic lymphadenectomy in comparison to patients who did not do pelvic lymphadenectomy even in patients who showed negative pelvic lymph nodes after lymphadenectomy. Preoperative staging with local radiology and for distant metastases is a must for every case for proper staging and management and using proper imaging modalities for diagnosis. Stage at presentation is the most important prognostic factor and patients with metastatic disease are the worst prognosis. This study concluded that it is in favor of the other studies and guidelines which recommend pelvic lymphadenectomy during surgical staging of uterine sarcoma cases
