Objective: To assess the impact of pregestational DM on fetal middle cerebral (MCA) and umbilical arteries (UA) Doppler indices to predict the possibility of adverse neonatal outcome. Methods: 120 patients were included and divided into two groups: Control group (group 1) included 60 healthy pregnant women and Study group (group 2) included 60 pregestational diabetic pregnant patients which furtherly subdivided into two groups according to HbA1C levels namely; Group 2a (included 30 diabetic pregnant women with controlled DM - HbA1C < 6.5%) and Group 2b (included 30 diabetic pregnant women with uncontrolled DM - HbA1C 6.5 %). UA, MCA Doppler indices (resistance index and pulsatility index) and Cerebroplacental Doppler ratio were measured for every patient. The recorded neonatal outcomes included neonatal birth weight, neonatal blood sugar, Apgar score at 1 and 5 min and neonatal intensive care unit admission
