This study was conducted to evaluate the primary stability of immediately placed two piece zirconium implants with autogenous bone graft through resonance frequency analysis. In this study 11 implants were placed in 3 males and 7 females with at least one remaining root in the anterior esthetic zone. The surgical procedures that were applied in this study were performed under local anesthesia. They included atraumatic extraction, drilling through the socket by subsequent drills according to the manufacturers instructions to the selected depth and width to achieve implant stability and maximum adaptation between the implant and socket wall. Stability of each implant was measured by Osstell ISQ immediately after implant placement and 4 months post the procedure before loading implant and abutment placement. No patient developed any post-operative serious complication that could affect implant success. All the 11 fixtures healed well with no infection or failure. We recorded only one implant that broke during placement without any excessive force or torque applied and it was taken out. The analysis of the effect of different variables on the implant stability showed that there was a strong correlation between primary stability and secondary stability
