Objectives & aim of work: to evaluate the role of ultrasound Shear-Wave elastography in assessment of diabetic peripheral neuropathy in the tibial nerve. Background: Diabetic neuropathy is the most common complication of diabetes. Early diagnosis of DPN is essential for prognosis and treatment because early treatment of DPN decreases both short-term and long-term morbidity. SWE elastography is a noninvasive and reproducible method for the precise evaluation of nerve stiffness. Methods: We studied 30 cases with diabetes mellitus and 20 healthy adults served as a control group. There were 19 females and 11 males diabetic patients having a mean age of 52 ± 9 years; the range of duration of diabetes was 8 years (1 - 28 years), with 11 females and 9 males healthy adults having a mean age of 33 ± 10 years. All selected cases were subjected to proper history taking, nerve conduction study, cross sectional area and shear wave elastography measurements of both posterior tibial nerves. Results: We found that tibial nerve stiffness is notably high at SWE in diabetic patients with DPN. Patients without DPN also had significantly higher nerve stiffness values than healthy control subjects. There is significant increase in the CSA among diabetic patients with DPN and without DPN in comparison with control subjects in the right side with borderline significance in the left side
