Objective: To study Measurement of fetal lung volume (FLV) and pulmonary artery resistance index could potentially be used to predict neonatal RDS at term pregnancy in patients undergoing elective Ceserean section of gestational age between 37 to 40 weeks. methods: a prospective cohort study as developed, in which 50 pregnant women that will have an elective ceserean section at gestational age 37 to 40 weeks were selected, underwent measurement 3d fetal lung volume by vocal and pulmonary artery resistance index measurement . then observing the percentage of the development of neonatal RDS. results: there was a direct correlation between the fetal lung volume (flv) and development of neonatal RDS .Acut-off for mean FLV 33.9 cm3 predicted the subsequent development of RDS with sensitivity 83.3 % and specificity 88.6 % conclusion: We concluded from our study that detection of fetal lung maturity in fetuses of GA 37 and 40 weeks of gestation of patients undergoing CS by non invasive predictor would be best carried out by doing 3 D measurement of fetal lung volume by VOCAL as they proved to be inversely proportional to RDS developed in neonates. Pulmonary artery resistance index proved to be with no significant value
