ObjectivesThe evaluation of the effect of pre-incisional infiltration of Bupivacaine hydrochloride in the skin and subcutaneous tissue at the planned Pfannenstiel incision site in ladies undergoing lower segment cesarean section regarding the control of postoperative pain and its impact on the patient early ambulation and breast feeding. MethodsSixtyof the pregnant women who presented to kasrAlainy from February till June (2019); and met the study inclusion criteria weredivided into two equal groups. In group A: the skin and subcutaneous tissue at the planned Pfannenstiel incision was infiltrated with Bupivacaine hydrochloride 0.25%, 0.7 mg/kg solution. And in group B (the control group): no local anesthetic skin infiltration was done, in both groups we studied the efficacy of pain control through the VAS system and time of first need for analgesia during the first 24 hours after cesarean delivery as well as time of earliest breast feeding.Results:theres a statistically significant difference between both groups regarding VAS score at the first six postoperative hours, also regarding thenumber of rescue analgesia doses needed postoperatively, andregarding the early initiation of ambulation.On the other hand, there was no statistically significant difference between the two groups regarding VAS score after 12 hours postoperatively,And there was also no statistically significant difference between the two groups regarding the early breast feeding initiation
