Our study included 40 patients presenting to Magdi Yacoub Aswan heart center with acute myocardial infarction eligible for primary PCI. They underwent first visit MRI after being clinically stable & follow up MRI was performed after 3.6 ± 0.8 months. Clinical, laboratory & PCI data were collected & processing of the MRI images was performed to assess the cardiac volumes & functions (in the first & second visit), hyperenhanced segments & analysis of tissue mapping images. Statistical analysis was preformed to suggest cut off values for tissue mapping for detection of the myocardial necrosis & determination of the possible prognostic role of the tissue mapping values. Our results suggested tissue mapping cut off values with reasonable sensitivity, specificity, positive predictive value, negative predictive value & accuracy. Such indices are noted to be of lower values in segments with MVO. Our study suggested T2 map cut off value for detection of myocardial edema of 53.2 ms with 80% sensitivity, 71% specificity, 74% positive predictive value, 78% negative predictive value & 76% accuracy. We also suggested cut off value for ECV for detection of myocardial necrosis of 36.4% with 78% sensitivity, 89% specificity, 88% positive predictive value, 80% negative predictive value & 84 accuracy
