Objective: To study the association between serum APOE level and APOE gene polymorphisms and idiopathic Parkinson{u2019}s disease. Subjects and Methods: Fifty-five patients with idiopathic PD and thirty healthy subjects were enrolled. Disease severity was assessed using Unified Parkinson{u2019}s Disease Rating Scale (UPDRS), Modified Hoehn and Yahr (HY) scale, and Schwab-England Activities of Daily Living scale. All participants were genotyped for APOE polymorphisms using polymerase chain reaction (PCR). Serum APOE level was also measured using enzyme-linked immunosorbent assay (ELISA). Results: Mean serum APOE level was significantly higher in PD patients compared to healthy controls. APOE f2/4 genotype was present in a significantly higher proportion in patients with PD compared to controls, however, no significant difference was found in the distribution of other APOE genotypes or APOE alleles between the two groups. f4 allele was significantly associated with a higher score on the {u2018}mentation, behavior, and mood section{u2019} of UPDRS compared to f2 allele. APOE f2 allele was significantly associated with a shorter disease duration compared to f3 and f4 alleles. Mean serum APOE level was significantly higher in patients presenting by predominant rigidity and bradykinesia compared to patients presenting by predominant tremors, but nodifference was found regarding the distribution of APOE genotypes. A significant positive correlation was found between serum APOE level and mean scores of {u2018}mentation, behavior, and mood section{u2019} of UPDRS, but not with the rest of PD rating scales mean scores ER -
