My cobacteria other than tuberculosis (MOTT) are globally recognized as pathogens especially in immune-compromised population. Species-level identification of MOTT is becoming increasingly clinically important. Patients and Methods: A total of 810 samples were involved and the positive culture isolates underwent Identification Test for MOTT identification then species identification by LCD array MycoDirect 1.7 and MALDI-TOF MS. Results: A total of 58 mycobacteria culture positive out of suspected 810 samples which were collected from Cairo University Hospital, Egypt. The prevalence of MOTT was 0.7 % among total samples and 10.3% among positive culture samples MOTT species identification was done by MALDI-TOF MS from strains isolated on Lowenstein Jensen. We performed the recommended protocol of heat inactivation and extraction. Also, Identification was simultaneously done by other techniques as LCD array MycoDirect 1.7, 16S rRNA and WGS for some MOTT isolates as we were hindered by financial issue. Our six MOTT isolates were identified by MALDI TOF MS as two M. fortuitum (33.3%) isolates, one M. abscessus (16.7%) isolate, one M. porcinum (16.7%) isolate, one M. gordonae (16.7%) isolate and one M. bacteremicum (16.7%) isolate. Matching to MALDI TOF MS identification we could perform identification to M. fortuitum by 16 sRNA which was identifiesd as M. neworleansense and it is closely related specied to M. fortuitum groupM.abscessus which was identified by MALDI TOF was also yielded by WGS. Also, LCD array MycoDirect 1.7 results for five strains with the MALDI TOF were concordance but failed to recognize M.bacteremicum
