Introduction: The aim of this study was to assess the correlation between acute encephalitis and cardiac dysfunction by using an advanced 3D speckle tracking echocardiography that seem to be superior to standard echocardiography to determine subclinical cardiovascular involvement in acute encephalitis & to investigate parameters that could predict cardiac dysfunction in acute encephalitic pediatrics. Methods: 84 children including 42 cases with acute encephalitis between 1month to 18 years and 42 age and surface area matched controls, underwent full history taking, clinical examination, andsubjected to conventional echocardiography and 3D Speckle tracking echocardiography which is able to discriminate the myocardial dysfunction in cases of acute encephalitis through assessment diastolic and systolic functions. Laboratory tests and imaging including CSFanalysis and culture, CT, MRI, EEG were derived from patients file. Results: Acute encephalitic patients had a myocardial dysfunction, the advanced echo parameters showed LVE/E' ratio (cases: 15.24± 1.68 vs. controls: 7.62± 2.39; p value < 0.001); RVE/E' ratio (cases: 12.11± 1.67 vs. controls: 19.65 ± 2.95; p value < 0.001); GLS (cases: 12.33± 1.37 vs. controls: 8.13± 2.01; p value < 0.001)
