Background The incidence of drug related problems (DRP) in intensive care unit (ICU) is very high compared to any other ward in the hospital. This was found to be related to the critical condition of admitted patients and the special ICU environment. The occurrence of DRPs is associated with negative clinical and economic outcomes as it increases morbidity, mortality and healthcare cost. Clinical pharmacists have an important role in identification and management of ICU DRPs because they have extensive knowledge in pharmacology (pharmaco-therapeutics, pharmacokinetics and pharmacodynamics). Due to the complexity of ICU cases, clinical pharmacist might not detect some important DRPs mainly due to lack of structured workflow and dependence on memory in case management. Objective This study was designed to evaluate the impact of using pharmaceutical care pathway including predetermined core measures and structured workflow on the detection and management of DRPs in the ICU. Methods The study was conducted in a general ICU in one of the public hospitals in Egypt and was divided into three phases: tool preparation phase, the control phase, and post-implementation phase.The tool preparation phase included the development of a core measures reference pathway, daily working scenario flow-charts and individual patient checklist
