Background Perianal fistula is a chronic recurrent inflammatory condition that may require repeated surgical intervention; hence adequate pre-operative radiological assessment is crucial for the disease outcome and preventing recurrence.Objectives The goal of this study is to assess the role of endoanal ultrasonography compared to MRI fistulography in evaluating the primary fistula's tract, internal opening, secondary extensions, and complications.Patients and Methods In this prospective study, 30 patients with perianal fistula were evaluated in diagnostic and interventional radiology department between September 2019 and May 2021.All the patients were subjected to endoanal ultrasound followed by MRI fistulography.Results: Intersphincteric fistula was detected in 18 cases by each modality, trans-sphincteric fistula in 10 cases by ultrasound compared to 11 cases by MRI , extra-sphincteric fistula in 1 case by MRI and no fistula was detected in 2 cases by ultrasound.The internal opening was detected in 28 cases by ultrasound compared to 24 cases by MRI. Secondary extension in 1 case by ultrasound compared to 2 cases by MRI and collections were detected in 9 cases by each modality
