Intervention: In case of bad skin condition, 2 staged procedure was done (first stage was temporary spanning external fixation and the second stage was ORIF) but in cases with good skin condition after resolution of edema open reduction and internal fixation was done directly.Results: sixteen patients comprised the elderly group (mean age 47.94 years) and sixteen patients comprised the younger group (mean age 29.22 years)(p< 0.001).beside significantly more tobacco use in the younger group, patient demographics and fracture characteristics indifferent. Infection (elderly 10% vs. young 18%, p=0.4), or malunion (elderly 3% vs. young 5%, p=1.0), nonunion (2.7% only in group B) and arthrosis (2.7% in group A and 5.8%in group B).Conclusion: Age > 40 years is not an independent predictor of surgical treatment failure of pilon fractures as defined by malunion, nonunion or arthrosis. Our results indicates that we should continue to treat elderly patients similar to their younger counterparts using ORIF ER -
