N2  - Objectives: To study the effect of primary knee osteoarthritis on the mental health status, and the relation between knee pain, stiffness, limitation of motion, deformity, and radiological changes and mental health status in primary knee osteoarthritis patients. Methods: Fifty osteoarthritic patients fulfilling classification criteria for primary knee osteoarthritis were recruited from the Rheumatology and Rehabilitation clinic; Kasr Al-Ainy University Hospitals &Fifty age and sex matched healthy individuals were included as a control group. Patients were subjected to full history taking, clinical examination and plain X ray both knees standing AP view scored according to the Kellgren and Lawrence (K-W) classification system .All participants responded to the hospital anxiety and depression scale (HADS) questionnaire.The Arabic version of HADS was used and explained in simple form. Results: The current study included fifty patients, twenty nine were females (58%) and twenty one were males (42%). Age of patients ranged from 45 to 80 years with a mean of (57 ± 9). The mean HADS scores was statistically significantly higher in patients group (16.52 ±7.380) than in controls group (11.80 ± 6.749) (p=0.001). The mean HADS A (Anxiety) scores was statistically significantly higher in patients group (8.66 ±4.32) than in controls group (5.3 ±4.25) (p<0.001). Anxiety was more frequently detected in patients (44%) than in controls (10%) (p<0.001). The mean HADS D (Depression) scores tended to be higher in patient group (7.86 ±3.79) than in controls group (6.5 ±3.32), and the frequency of depression was non-significantly higher in patients group (32%) than in control group (14%), (p=0.059 and p=0.069 respectively). Nine patients had abnormal HADS A (Anxiety) only, three patients had abnormal HADS D (Depression) only, and 13 patients had abnormal HADS A and D.There is significant association between HADS and each of female sex of patient (p<0.001), inactivity stiffness (p=0.039), locking of knees (p=0.016), crepitus (p=0.018), limitation of motion (p=0.039), and stiffness duration - p=0.037
