Hemiparesis is the most frequent neurological deficit after stroke. Was to investigate the effect of Biodex balance training on balance in hemiparetic patients. Forty ambulant hemiparetic patients from both genders (31 males and 9 females) participated in the study when the patients were clinically stable after 6 months of attack and classified randomly into two equal groups (Group (A) and Group (B) ). Both groups received a conventional physical therapy treatment. In addition, Group (B) received balance program using the Biodex Balance System including dynamic balance training and dynamic limits of stability training. The treatment program was conducted three times per week for 12 weeks and the patients were assessed for the scores of tinetti balance test and biodex balance system. These measures were recorded three times, before the application of the treatment program, at the middle of application (after 6 weeks) and after the end of treatment program (after 12 weeks). The results of this study showed that there was significant improvement in Tinetti Balance Test in Group (A) and Group (B) and the improvement was higher in the Group (B) than Group (A). For Group (B) there was higher significant improvement in dynamic balance and dynamic limits of stability. It was concluded that this balance program is more effective in improving balance than conventional physical therapy alone, in hemiparetic patients following stroke
