Background In face of catastrophe of sepsis, early antibiotics are the single most efficient intervention. Current diagnostics modalities are hampered by delayed results, poor sensitivity and specificity. So there urgent need for biomarker that is more fast and reliable. Aim of the work Evaluation of role of Presepsin as early marker of sepsis in Pediatric Cardiac Intensive Care Unit (PCICU) patients. Methodology The study included 30 cases and 15 controls selected from infants with congenital heart disease (CHD) admitted to PCICU without sepsis and observed tell sepsis onset. In cases the sample at sepsis onset and the one before it, and in controls any sample were taken and analyzed for presepsin using ELISA technique. Results Presepsin was found to havepoor diagnostic. Furthermore it was revealed that presepsin level doesn{u2019}t correlate with. Conclusion & recommendation Presepsin was found to be non-beneficial as both as diagnostic and prognostic tool in pediatric sepsis, but this needed to be reassessed in larger group of patient. Furthermore, more studies needed to be done in area of presymptomatic sepsis diagnosis as well as evaluating role of presepsin and other biomarkers in different study populations and in different causes of infection
