Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Orthopedic Surgery Summary: The gradual increase in the trauma due to motor car accident relatively increase the high energy trauma including acetabular fractures, posterior wall acetabular fractures is the most common type among acetabular fractures, due to the complex shape of the acetabulum it represents a major challenge to any orthopedic surgeon.₍¹₎The optimal treatment for displaced posterior wall acetabular fractures is surgery, and various fixation methods such as reconstruction plates and spring plates have been introduced.Kocher-Langenbeck approach is standard for posterior wall fracture. ₍²₎ Only 30% of these fractures involve a single large fragment. The majorities are multifragmentary or have areas of impaction. Reconstruction of these comminuted or impacted posterior acetabular wall fractures is difficult; the problem is compounded when the posterior wall involvement exceeds 50% of the surface area and leads to hip joint instability.The usual treatment method as described in literature includes articular fragment reduction and bone grafting with buttress plating to support the elevated and comminuted fragments.Some surgeons prefer to buttress the small fragments with spring plates. ₍⁵₎
