Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Pediatrics Summary: Packed red blood cells (pRBCs) transfusion has become a significant therapeutic intervention in the pediatric intensive care unit (PICU). However, transfused blood products demonstrate transfusion related immunomodulation (TRIM) that may worsen patient outcomes that is also more associated with blood storage time. Objective: Our observational study aims to demonstrate immunomodulatory effect of filtered pRBCs transfusion on adaptive immune response by measuring serum interleukin-2 (IL-2). Patients and methods: Thirty critically ill children admitted to PICU, Cairo University, were selected as the study group. Serum interleukin-2 level was measured before and 18 ±6 hours after transfusion of filtered pRBCs. Patients were divided according to storage of blood transfusion into fresh (<14 days) group (n=15) and old (>14 days) group (n=15). In addition to 30 apparently healthy children served as a control group (non- transfused). Samples analysis was done using enzyme linked immunosorbent assay (ELISA) in the clinical pathology and chemistry department in al Kasr al Ainy laboratory. An informed written consent was obtained from parents before enrollment
