Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Radio-diagnosis Summary: Multiparametric MRI (mpMRI) combines anatomic T2W with functional and physiologic assessment, including diffusion weighted imaging (DWI) and its derivative apparent diffusion coefficient (ADC) maps, dynamic contrast enhanced (DCE) MRI, and sometimes other techniques such as in-vivo MR proton spectroscopy. mpMRI, have substantially improved diagnostic capabilities for addressing the central challenges in prostate diseases and lead to improving accuracy of diagnosis, prostate Imaging reporting and data system (PI-RADS) is a scoring system that aims to enable consistent interpretation, communication and reporting of prostate multiparametric MRI (mpMRI) findings. Aim of study: To assess the upgrade of the multi-parametric MRI prostate as well as the role of PIRADS as a reporting and grading system in prostatic diseases. Patients and methods: Our study was prospectively carried on 62 male patients with prostatic symptoms (between February 2015 to October 2016), Our study was performed in the radiology department of the Kasr Alainy hospitals, Egyptian national cancer institute. Patients were referred from the Outpatient clinic of the urology department
