Background: Polycystic ovary syndrome (PCOS) is a complex disorder which affects the rate of fertility. Purpose of the study: The purpose of this study was to determine the effect of bee venom (BV) phonophoresis in treatment of obese PCOS women. Participants and methods ;forty six obese PCOS women with age range, 18- 39 years and body mass index (BMI> 29) kg/m2 were recruited in this study. They were divided randomly into two equal groups; group (A), 23 females received phonophoresis with BV topical application in addition to low caloric diet (1200-1400 kcal /day) and group (B) (control group) who followed low caloric diet (1200-1400 kcal /day) and sham ultrasound sessions without BV. Assessment was done by biochemical analysis of luteinizing hormone (LH), follicle stimulating hormone (FSH), LH/FSH ratio, and progesterone (P4) levels, measured before, after 7 weeks, and after 14 weeks of treatment. Results :The current study revealed that there was significant increase in P4 and significant decrease in LH and LH/FSH around the middle of study in group A and for both groups by the end of study compared to mean value before treatment. There was non-significant change in FSH around the middle of the study and by the end of study for both groups. Comparison between groups revealed that there was no significance difference between both groups by the middle and end of the study except for P4 that showed significant increase in BV group in the middle of the study. Conclusion BV phonophoresis with diet and physical activity have beneficial effect in treatment of obese PCOS women
