Background: Pediatric bow knee (genu-varum) is a very common orthopedic presentation, different types of surgical procedures have been described for correction of pathological type of this presentation. In this study we have evaluate the outcome of percutaneous high tibial osteotomy and casting in genu varum deformity. Objectives: This is study aiming to evaluate the outcome of percutaneous high tibial osteotomy and casting for genu varum deformity. Patients and Methods: A randomized controlled (closed envelope) prospective interventional study aiming at evaluation of new technique in treatment of paediatric genu varum (percutaneous osteotomy). 20 cases (35 limbs) were included in our study, 14 males and 6 females,15 bilateral and 5 unilateral. All patients underwent surgery at either Abo Al-Rich Specialized Pediatric Hospital or The National institute of Neuromotor System between 2018 and 2020. Results: The study showed significant radiological improvement in mLDFA, MPTA and mechanical axis deviation post-operative. Patient show significant clinical improvement according to Böstman score. Conclusions: This surgical procedure is reliable method of correction of genu-varum deformity in children before maturity with some precautions to reduce rate of complications reported with other methods
