Background: Cesarean section is surging worldwide. For an extended period of surgical practice, scalpel has been well-known as a gold-standard tool for making surgical incision.The diathermy, electrocautery, is a substitute. Aim of the work: to compare between diathermy and steel scalpel incision in obese patients in terms of wound related blood loss, operation time, postoperative wound complication rates, postoperative pain, and scar character. Patient and methods: This study comprised 300 pregnant obese women in childbearing period who were chosen from the department of gynecology and obstetrics in Poulak Eldakrour General Hospital during the period of research from October 2019 to March 2020. In both groups, skin was incised till white line using scalpel then the studied cases had been divided into two groups: Group (I) that included 150 patients had their incisions completed using the surgical scalpel and group II that included 150 patients their incisions completed using diathermy. Results: There was a statistically significant difference in regards to the incision time (P <0.001), operative time (p -0.045) between scalpel and diathermy groups, which was less in the latter. There was no statistically significant differences between scalpel and diathermy groups at 1, 4 hours postoperatively, whereas There was significant decrease in the postoperative pain scores after 6, 12, 24 hrs of operation (p {u2264}0.001)
