Introduction: The adrenal gland is a retroperitoneal organ which is located under the diaphragm. The adrenal tumors are not infrequent and their incidence increased after the use of Computerize Tomography (CT). The laparoscopic approach has become the favored technique to remove tumors of the adrenal gland. Aim of work: Comparing open adrenalectomy versus laparoscopic adrenalectomy in management of benign adrenal masses as regards safety and efficacy. Patients and methods: retrospective cohort study which was performed on 83 patients who underwent adrenalectomy for benign looking adrenal neoplasm at National Cancer Institute (NCI), Cairo University along a period from January 2014 till January 2020. Data were collected from files included; age, sex, side of adrenal lesion, size of adrenal lesion, size of adrenal gland, surgical procedure, and biochemical tests (serum metanephrins-urine vanillymandelic acid (VMA)- serum aldosterone- serum cortisone- serum Na and K). Results: Fifty seven patients undergo right adrenalectomy (68.7%) & 26 patients undergo left adrenalectomy (31.3%).In our study 38.6% had functioning lesions including pheochromocytoma (31 patients) and hypercortilism (1 patient), while 51 patients (61.4%) had non-functioning lesions.There was significant statistical difference between both groups (open &laparoscopic) for the pathological size of the lesion (P value = 0.014). Among 51 patients undergone open adrenalectomy, no deaths had occurred post-operative, however among 32 patients undergone laparoscopic adrenalectomy, one patient died post-operative
