Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Pediatrics Summary: Background: Urinary tract infections (UTIs) are a common and important clinical problem in childhood. Despite the presence of simple and reliable methods of screening of UTI, the diagnosis of UTI is often overlooked due to difficulties in obtaining urine sample from children. Aim: The aim of this study was to assess the validity of urinary dipstick test in comparison to standard urine culture technique in diagnosis of UTI, and to know the prevalence of UTI among febrile preschool children patients and methods: This cross sectional study was done on febrile preschool children, attending pediatric outpatient clinic at bulaq Eldakrour general hospital. All children were subjected to full medical history, complete physical examination, and urine specimen collection, dipstick analysis by using leucocyte esterase and nitrite detector as well as urine culture and antibiotic sensitivity test. Results: This study was conducted on 107 children 70 (65.4%) girls with mean of age 3.8years ± (0.9) SD. Results of the current study show that children with positive Leucocyte esterase were 11(10.3%), positive nitrite were 7(6.5%), (5 cases of them had both leucocyte esterase and nitrite positive). Leucocyte esterase sensitivity was (66.7%), specificity (94.9%), positive predictive value (PPV) (54.5%) and negative predictive value (NPV) (96.9%). Nitrite sensitivity was (44.4%), specificity (96.9%), PPV (57.1%) and NPV (95%). When both tests were positive, sensitivity was (33.3%), specificity (98%), PPV 60% and NPV 94.1%
