Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Community Medicine Summary: Background: Quality of life (QoL) assessment is increasingly recognized as a key outcome in both clinical and research settings in the pediatric population with end-stage renal disease (ESRD) Objectives: this study aimed at assessment of quality of life among children and adolescents with ESRD either undergoing Hemodialysis or had renal transplantation therapy. This was done through measuring different essential life aspects including: physical, social, emotional and school functioning as well. Methods: A case-control study was conducted in Abo El-Reish pediatric hospital using parent/ child reports of generic module for QoL assessment: PedsQLTM inventory Version 4 for both cases and controls. In addition, a disease specific module: PedsQLTM ESRD version 3 was used for ESRD cases (dialysis and transplantation subgroup). A 55 ESRD cases from nephrology pediatric unit were enrolled in this study. As well as an 86 controls were also enrolled in the study from Abo El-Riesh attendees seeking acute illness at outpatient clinics. Results: There was a statistical significant difference between ESRD cases and controls regarding all aspects of QoL. As it revealed that total QoL mean score was 58.4±15.3 SD for ESRD cases while that of controls was 86.8 ± 10 SD. Additionally, all individual QoL domains as physical, social, emotional and school dimensions were significantly worse in ESRD cases than controls. School attendance was greatly affected. As 25% of ESRD were not enrolled into school. The majority of them were among dialysis subgroup. Furthermore, there was no statistical difference between dialysis and transplantation subgroups regarding QoL dimentions except for renal disease and treatment related problems. As transplantation group had better scores than dialysis group. Spearmans{u2019} correlation between child and parents{u2019} scores showed significant positive moderate correlation among different studied groups. While intra-class correlation showed that there was moderate to good agreement between both child and parents total mean scores as well as individual domains. Conclusion and recommendations: ESRD and its treatment modalities (dialysis or transplantation) are affecting negatively all aspects of quality of life. So Optimal care of the pediatric ESRD patient must now incorporate QoL assessment and management
