Background: Heart failure is one of the most frequent causes of death worldwide. Recently, there has been emerging evidence suggesting that micronutrient dyshomeostasis is associated with HF Objectives: to measure zinc level in pediatrics with congenital acyanotic heart diseases, with and without heart failure in comparison to controls, and to investigate the relationship between serum zinc level, severity of heart failure, and medications used to treat heart failure. Methods: cross sectional descriptive study was conducted on 100 patients with congenital acyanotic heart diseases. Serum zinc levels in 50 patients with heart failure and 50 patients without heart failure were compared to serum zinc levels in 50 age and sex matched control group. History taking, clinical examination for growth assessment and stage of heart failure, measurement of serum zinc level and transthoracic color Doppler Echocardiography were done. Results: Serum Zn level was significantly lower in patients (76.71±19.74) than controls (132.33±20.96), p value <0.001. It was lower in heart failure patients (66.54±17.08) than those without heart failure (86.89±16.86) with p value <0.001. There were significant differences in serum zinc levels in regard to NYHA and ROSS classifications. There was no significant correlation between serum zinc level and echocardiographic parameters Conclusion: Serum zinc level was significantly low in congenital acyanotic heart pediatric patients and significantly related to the severity of heart failure but didn{u2019}t correlate with diuretics use or angiotensin converting enzyme inhibitors
