Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Orthopedic Surgery Summary: The resulting pain and loss of function after femoral neck fractures poses a socioeconomic problem in young patient that limit their productivity. The role of surgery is to reduce the consequences that endangering patient life and to achieve the highest functional outcome and decrease the possible complications of prolonged recumbency. Head preservation is the mainstay of management in younger patients with neglected or nonunited intracapsular fracture neck of femur. Osteosynthesis in a femoral neck nonunion is challenging because of the fracture morphology and augmentation techniques in the form of grafting or osteotomies are often required. A Femoral neck fractures nonunion represents a biomechanical failure rather than a biological failure alone. So restoration of biomechanics at the proximal femur and at the nonunion site is considered an integral part of any surgical procedure aimed at achieving successful osteosynthesis
