Fat containing lesions of the thorax have a wide range of differential diagnosis. Those lesions could involve the lung parenchyma, different mediastinal compartments, daiphragm, and chest wall. The cause of such fat containing lesions may be determined by means of fatty lesion site, distribution , content, shape , size and associated CT findings and knowledge of the associated clinical features. MDCT is currently the imaging modality of choice in diagnosis of different fat containing chest lesions, being superior to chest radiography in demonstrating the presence and extent of chest abnormalities. Our study involved reviewing the MDCT results of 20 patients of different complaints and different diseases who were referred to kasr AL- Aini radiology department. Using MDCT findings, we were able to identify, detect, and localize various types of chest fat containing lesions to aid diagnosis and to discuss the differential diagnosis and the pathogenesis. So, we concluded that the role of MDCT imaging in diagnosis and evaluation of different chest fat containing lesions is central, being accurate and non-invasive
