There are many confounding factors that contribute to the increased incidence of PONV after bariatric surgeries, including of the gastroesophageal junction due to hiatal hernia and obesity, direct gastric irritation by surgical trauma, blood, and secretions, laparoscopic surgery itself, increased surgical duration of more than one hour, and increased requirements for intraoperative hypnotics and perioperative use of opioids. )3,4)
Consequently, a variety of pharmacological agents have been employed to prevent and treat PONV, including dopamine receptor antagonists (such as metoclopramide) (5); histamine antagonists (such as prochlorperazine and Cyclizine) (6), anticholinergics (such as scopolamine) (7), serotonin antagonists (such as ondansetron) (5), dexamethasone (8), and neurokinin antagonists (such as Fosaprepitant) (9). However, these medications have been linked to a number of undesirable side effects, such as excessive sedation, high blood pressure, dry mouth, hallucination symptoms, and extrapyramidal effects. (10, 11)
The use of multimodal antiemetic agents represents a better choice for preventing PONV than using a single agent, as using different mechanisms enables optimal perioperative outcomes and facilitates fast-track bariatric surgeries. Furthermore, systemic corticosteroids possess
