Clarithromycin-based eradication therapy regimens represent the gold standard in management of H. pylori organism. Clarithromycin resistance is the main cause of failure of eradication therapy which may be due to multiple genetic mutations, single or combined together. Aim of work: Determining the prevalence and the genetic background of clarithromycin resistance of the H. Pylori strains isolated from gastric biopsies in Egyptian patients. Patients and methods : Gastric biopsies were obtained from 100 egyptian patients having upper gastrointestinal manifestations. Biopsies underwent PCR for detecting H pylori organism then PCR-RFLP to detect clarithromycin resistance. Results : H pylori was detected in 70 patients, of them, 35 cases were resistant to clarithromycin CONCLUSION : PCR and PCR-RFLP techniques are now being used as a reliable way for detection of H pylori organism and clarithromycin resistance gene mutations, respectively.
