Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Pathology Summary: Background: Meningiomas are the second most common primary neoplasms of the central nervous system. Several studies have reported a relation between MMP9 expression and histological grade, subtype, invasiveness and recurrence of meningiomas. Objectives: Analysis of MMP-9 expression in a series of meningiomas of different histotypes and histological grades and assessment of its correlation with various clinico-pathological indicators. Results: Positive immunostaining for MMP9 was encountered in (43/50) 86% of meningiomas,44%showed high MMP9 expression while 42% cases showed low MMP9 expression.Statistical analysis revealed the existence of a significant association between high MMP-9 expression and high histological grade (P < 0.001),recurrence of the tumors (P = 0.021), brain-invasive growth (P<0.001), large tumor size(>5cm)(P<0.001)& certain histological subtypes (MMP-9 expression was highest in atypical, malignant, chordoid and clear cell variants)(P=0.001). Conclusions: A strong direct relation between MMP9 expression and the grade of meningiomas was observed. MMP-9 expression behaves as a negative independent prognostic factor for the risk of tumor recurrence. So Inhibition of MMP-9 may be a new therapeutic strategy to prevent recurrences of meningiomas, particularly the high-grade type
