Summary: The main objectives of the current randomized controlled clinical trial was to evaluate the effect of platelet rich fibrin (PRF) versus free gingival graft (FGG) in preservation of hard and soft tissue volume of extraction socket/ alveolar ridge, in terms of the soft tissue profile for assessment of the soft tissue changes and crestal bone level for assessment of bone changes, after atraumatic tooth extraction and socket preservation procedure. The buccal walls of the extraction sockets were intact without any dehiscence or fenestration defects. Atraumatic extraction was performed for the hopeless tooth and application of either PRF membrane or FGG was done after curretting the sockets carefully. Impressions were taken for patients at baseline (before extraction) as well as 6 months postoperative to obtain cast models for optical scanning to assess the postprocedural changes in soft tissue profile, also CBCTs were taken twice (at the aforementioned time intervals) for determination of crestal bone levels, and were superimposed to measure the postoperative crestal bone level changes. Patient‟s satisfaction was assessed through a questionnaire for each patient as well as pain was assessed one week postoperative via the Visual Analogue Scale (VAS); Pain was considered as a primary patient-centered outcome as it affects the patient‟s quality of life. The study resulted in reduced pain scores in favour of the intervention (PRF) group. Insignificant difference (p=0.528) in mean Soft tissue Profile changes was found between PRF and FGG. Regarding the crestal bone level changes, the intergroup comparisons showed insignificant differences, except for the average buccal-lingual changes, which was significantly higher in the FGG group than in the PRF group; however the percentage of change in crestal bone level for the average buccal-lingual was also statistically insignificant. As per the demographics (age, gender), the differences between the two groups were statistically insignificant.
