Aim: The aim of this randomized clinical trial (RCT) was to evaluate the adjunctive benefit of papillary extended Connective Tissue Graft (PeCTG) to Connective Tissue Graft (CTG) with coronally advanced flap (CAF) for the treatment of gingival recession Methodology: Twenty-eight patients were randomly assigned to test group (CAF+PeCTG) or to control group (CAF+CTG). Clinical parameters as recession depth (RD), recession width (RW), distance from contact point to tip of papilla (DCP), keratinized tissue width (KTW) and gingival thickness (GT) were measured at baseline, 3 months and 6 months. The root coverage esthetic score (RES) and postsurgical patient satisfaction were evaluated at 6 months. Results: At 6 months follow up; both groups showed statistical significance at all parameters and there was no statistical significance between both groups. The test group and the control group showed mean root coverage of 59.52% and 45.23% and complete root coverage of 28.57% and 21.42% respectively Conclusions: This randomized controlled trial demonstrated that CAF+PeCTG might represent a promising method for root coverage, notably for papillary gain in Miller Class III gingival recessions
