Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of Neurosurgery Summary: Management of recurrent giant pituitary adenomas is challenging and debatable. We conducted this study to compare between the transcranial and the transsphenoidal approaches as regards the extent of resection, clinical outcome, morbidities and mortalities. The study included 21 patients; 9 in the transcranial group (Group A) and 12 in the transsphenoidal group (group B). The transsphenoidal approach was associated with better extent of resection, better rate of visual improvement and endocrinological remission and lower rate of post-operative morbidities and mortalities; However, the difference was not statistically significant. We concluded that the transsphenoidal approach is the first option for management of these cases; however, the transcranial approach is still needed for some selected cases
