Objective: To evaluate the effects of an ultrasound guided Transmuscular Quadratus Lumborum Block (USG-QLBTM) used for postoperative pain relief by comparing the efficacy of two different volumes/concentrations of local anesthetic bupivacaine in patients undergoing unilateral inguinal hernia repair. Methods: This randomized study enrolled patients undergoing unilateral inguinal hernia repair. They were randomized in to two groups: Group BD received a volume of 30 ml LA (75 mg bupivacaine+15 ml saline solution) and Group B received a volume of 40 ml LA (50 mg bupivacaine+30 ml saline).The duration of postoperative analgesia, requirement for postoperative rescue analgesics, patient satisfaction scores, postoperative complications, and postoperative pain as measured by a visual analogue score at 10 min, 30 min, 60 min, 12 hrs, 24 hrs and 48 hrs were recorded. Results: A total of 32 patients were enrolled in the study. There were no differences between the two groups with respect to demographic characteristics, postoperative opioid consumption and hemodynamic parameters. The duration of postoperative analgesia and patient satisfaction scores were all significantly better in group B who received a larger volume but a lower concentration of local anesthetic solution compared with group BD
