Background: Osteoarthritis is recognized as an important source of disability and handicap which leads to considerable socioeconomic costs due to medical ,surgical intervention and frequent absence from work . Purpose of the study: To investigate and compare between the efficacy of shockwave therapy versus platelet rich plasma injection in management of knee osteoarthritis. Methods: Forty-five (males and females) Patients were divided into three groups of 15 patients each, aged from 40 to 65 years. Group (A) received radial shock wave therapy one session per week for a total of four sessions plus conventional physical therapy program. Group (B) received intra-articular platelet rich plasma injection plus conventional physical therapy program. Group (C) received the conventional physical therapy program alone in the form of strengthening for quadriceps, stretching of the hamstring and calf muscles , ultrasound and hot pack. All patients were evaluated pre and post treatment for knee pain severity by the visual analogue scale and functional disability using WOMAC scale and Rang of motion using laser goniometer. Results: There were significant differences in all all groups (A,B&C) in all variables .There were significant difference in both group (A&B) more than group C in all variables (P=0.0001) , There was no significant difference between Shock wave therapy and intra-articular platelet rich plasma injections as regard to pain (P=0.97), functional disability (P=0.347) and range of motion (P=0
