Background: Napsin A is expressed in many normal tissue including type II pneumocytes, alveolar macrophages, renal tubules and exocrine glands and ducts in the pancreas. It was postulated that Napsin A was more sensitive than TTF-1 for primary lung adenocarcinoma versus all tumors, excluding kidney, independent of tumor type. In 2003, using in situ hybridization, Ueno et al were the first to report napsin A expression in a renal cell carcinoma. While kidney tumors are the 9th most common cancer in men, and 14th most common cancer in women, with ccRCC account for 65-70% of all renal cancers. Aim: In this study we assessed the percentage of expression of napsin A in some types of primary parenchymal renal malignant neoplasms in adults. Material and Methods: This study tested the expression of napsin A in 53 cases of primary renal parenchymal neoplasms from adult patients. where the tumors included 17 ccRCC, 14 chRCC and 22 PRCC. The cases were obtained through retrieval of archival paraffin blocks (dating from 2010 to 2018), with established diagnoses, from the pathology Department, Faculty of Medicine, Cairo University, national kidney institute and a private laboratory. Results: Napsin A expression was detected in ccRCC 6/17 (35.3%), with 3/17 (17.6%) showing positivity <5%, and 3/17 (17.6%) showing positivity >5%, Also in PRCC in 14/22 (63.6%), with 2/22 (9.1%) showing positivity <5%, and 12/22 (54.5%) showing positivity >5%. Regarding PRCC type 1, 6/6 (100%) showed positivity >5%
