Background: Diabetes mellitus (DM) in pregnancy is one of the most common indications for fetal surveillance. This reduces the burden of perinatal mortality and long-term morbidities, while also avoiding unnecessary obstetric interventions. Doppler ultrasound in high-risk pregnancies improved a number of obstetric cares out- comes and was helping to reduce peri- natal deaths. There is consensus that its use significantly reduces perinatal mortality as well as iatrogenic prematurity and its complications Study Objective: To determine and compare the diagnostic performance of Doppler sonography of Umbilical Artery (UA), Fetal Middle Cerebral Artery (MCA), Ductus Venosus (DV) & Umbilical Vein (UV) for prediction of Adverse Perinatal Outcome among Diabetic Pregnant Patients.Design: A prospective cross sectional observational study Methods: 40 pregnant women with DM (Pregestational DM(PGDM) and Gestational DM (GDM)) diagnosed clinically were included in the study, from April 2021 to October 2021. All the patients underwent accurate color doppler velocimetry examination. The study population was divided into two groups re Outcome variables were intrauterine, and early neonatal death, admission to the neonatal intensive care unit and the duration of treatment, Apgar score below 7 at 5 minutes, cesarean delivery for fetal distress, gestational age at delivery, neonatal birth weight, IUGR and macrosomia
