Thirty two patients were recruited from Outpatient Clinic of Prosthodontic Department, Faculty of Oral and Dental Medicine, Cairo university. The patients had bilateral mandibular distal extension edentulous ridge (Kennedy Class I) opposing maxillary complete dentition or restored maxillary arch. The present study was designed to compare the effect of using extra-coronal attachments versus clasp designed removable partial denture of Kennedy Class I on the Oral Health Related Quality of Life (OHRQoL) and the biting force. The OHRQoL was measured using the shortened version of the Oral Health Impact Profile (OHIP-14 questionnaire) and the biting force was measured using iload star sensor device. All patients on both groups were instructed to fill the questionnaire after one week of insertion, 3 months, 6 months and 12 months as well as their biting forces were recorded. The data were collected, tabulated, statistically analyzed. The results revealed no statistically significant difference between the two types of prosthesis in all the seven dimensions of the questionnaire except for the psychological disability. The total OHIP-14 score revealed a statistically significant decrease in case of PA-RPD compared to C-RPD group
