Postmenopausal bleeding (PMB) accounts for 5% of all gynecologic office visits. PMB must always be investigated because many causes are premalignant or malignant. The current study is designed to explore the value of three dimensional power doppler ultrasound for diagnosis of endometrial cancer in correlation to postoperative histopathology in cases of PMB. 40 cases, recruited from the outpatient gynecological clinic, Kasr Elainy Hospitals, were included in the study. All cases presented with PMB. All patients were subjected to 3D transvaginal power Doppler ultrasonographic evaluation of the endometrium, and the results were correlated to the histopathological picture of the endometrium. The age ranged between 48 and 73 years with a mean of 56.77 years. They had a mean parity of 3.12 . The most common endometrial histopathology was atrophic endometrium (10cases, 25%) then endometrial cancer (9 cases, %) then endometrial polyps (8 cases, 22.5%) and endometrial hyperplasia (8cases, 22.5%) then submucous myoma 5 cases,12.5%
