The present study was conducted on 15 patients toassess the incidence of schniderian membrane perforation, the amount of bone height gained and implant stability following computer guided sinus floor elevation versus conventional sinus floor elevation using lateral window technique. Graftless tenting technique was the technique used in both groups.15 patients with 20 sinuses (9 males, 6 females) were divided into two groups, group A for computer guided sinus floor elevation and group B for conventional floor elevation. A total of 35 implants were inserted in both groups withdiameters ranged from 3.7 to 4.8 mm according to the clinical situation. To precisely locate the lateral osteotomy borders for group A, a prototype cutting guide using FDMA was used to define accurately the lateral window of the maxillary sinus and to precisely locate the sinus septa if present, 3D CT imaging and computer software were used for fabrication of the guide to facilitate the sinus lifting procedure. The fabricated guide was fixed intra-operative with mini screw prefabricated during planning to not interfere with the implant position
