The purpose of this study was designed to compare between the efficacy of Flutter device versus active cycle of breathing technique on arterial blood gases in COPD patients. Methods: Forty male patients with moderate to severe level of COPD (FEV1/FVC<0.7), were included in this study their age was > 40 years old. They were selected from El-Demrdash hospital, they were assigned into two groups equal in number: Group (A): received Flutter device for 4 weeks, 3times/ week, and Group (B): received ACBT for 4 weeks, 3times/ week. Evaluation was done for all patients by blood gases analysis test, 6minute walk distance and COPD Assessment test (CAT), all these measures were taken before starting and after finishing treatment program. The results of this study revealed that there was significant difference between Flutter and ACBT in these parameters (PH, SaO2, PaCO2, PaO2, 6MWD, CAT). There was significant increase in all the previous parameters in Flutter group more than ACBT group. Group (A) showed a statistical significant improvement in ABG parameters (PH, , SaO2, PaCO2, PaO2,), 6MWD and CAT that was (0.5%, 3.3%, -15.1%, 13.2%, 23.1% and -28.6%) respectively, more than the improvement in group (B), that was (0.1%, 1.1%, -7.2%, 4.5%, 10.1% and -15.0%) respectively
