Dissertation note: Thesis (M.Sc.) - Cairo University - Faculty of Medicine - Department of Radio-diagnosis Summary: Preterm neonates have a common complication which is failure of postnatal closure of the ductus arteriosus (DA) that may develop clinical signs of a patent DA (PDA) that become hemodynamically significant and require therapy. Blood flow to the vital organs decreases and changes in cerebral blood flow occur and can lead to brain injury. This study aimed at measuring the cerebral hemodynamic effects of PDA using cranial doppler ultrasonography and its relation to the prognosis. We performed a prospective cohort study included 71 preterm neonates were evaluated by echocardiography for the presence of PDA then clinical parameters assessment and trans cranial doppler of the anterior cerebral artery were done in the first 24 hours of life then follow up after 72 hours and divided into two groups hemodynamic significant PDA (HS PDA) and hemodynamic insignificant PDA (HIS PDA) based on clinical deterioration after 72 hours. We found that the resistive index (RI) of the anterior cerebral artery was strongly significant in detection of clinical deterioration of the hemodynamic significant cases
