Introduction Egypt- Ministry of Health (MOH) surveillance system provides important data for notifiable communicable diseases (CD). However, there are necessities for proper utilization of data for decision- taking to reduce the attack rates. Objectives Develop modules of surveillance reports that identify: the main CD with high attack rates throughout the period 2006-2013, by Egypt governorates, seasonal variations, age, gender and trend. Methods Data from the MOH-Epidemiology and surveillance unit 2006-2013 were submitted to analysis to estimate the attack rate by Egypt governorates. The main 15 notifiable CD were identified, and presented by age, gender and trend. Risk index expressed as average attack rate for each CD by governorate (2006-2013) was used to develop Choropleth maps for each of the 15 CD. The magnitude of the risk index was calculated from ranking score to categorize governorates into high, medium and low risk index for each 15 CD. The summaries of information were organized into six brief reports to be directed to policy makers for evidence-based interventions
