Dissertation note: Thesis (Ph.D.) - Cairo University - Faculty of Medicine - Department of E.N.T Summary: Background: Although the majority of the maxillary pathology can be eradicated through the enlarged middle meatus antrostomy (MMA), some patients have extensive disease that is difficult to cope with. Aim: To compare between the efficacy of the inferior meatus osteo-mucosal posteriorly based flap approach versus conventional middle meatal antrostomy in dealing with various maxillary sinus pathology. Material and methods: This prospective study was conducted on 60 patients with various forms of maxillary sinus pathology, recruited from the out-patient clinic in Kasr Al-Aini hospital from the period of March 2017 to June 2018. The patients were randomized in to two equal groups: 30 patients underwent inferior meatus osteomucosal posteriorly based flap approach with or without MMA in group (A), 30 patients underwent conventional MMA in group (B). Both groups are evaluated intraoperatively regarding operative time, surgical access of the maxillary sinus walls and residual detection. Postoperative outcomes included endoscopic evaluation after 2 and 8 weeks, CT after 3 months and detection of possible complications
