The research examines the effect of medication review and patient counseling in improving patient knowledge and reducing medication related problems. 69 adult patients were selected and they were asked to bring all the medication including prescribed and non prescribed drugs to the researcher's community pharmacy. The researcher assessed the patient's knowledge using questionnaire which covers all aspects of medication regime
