Background: Myofascial Pain Syndrome (MPS) is a well-known painful musculoskeletal disease characterised by myofascial trigger points and tense bands in the muscles, local spasm, stiffness, and joint restrictions. Purpose of the study: This study was conducted to compare the effectiveness of phonphoresis and low level laser therapy(LLLT) on trigger points of upper trapezius muscle. Materials and Methods: Forty five patients with myofascial trigger points in upper trapezius muscle participated in the study with their ages were ranging from 20-40 years, for one month and they were randomly assigned into three groups. Group A: Received LLLT, Group B: Received phonophoresis, Group C: Received LLLT and phonophoresis with traditional treatment for all the three groups. Cervical range of motion were measured by cervical range of motion device and pain intensity by pressure algometer. Results: 3{u00D7}2 Mixed design MANOVA) revealed that there was significantly increase in neck extension, flexion, side bending, rotation (P=0.0001; P<0.05) post treatment compared to pre-treatment within groups, this significant increase in cervical range of motion post-treatment was in favor of both phonophoresis and laser group than phonophoresis alone and laser alone and for pain intensity, there was a significant decrease in pain in group C as compared with other groups post-treatment
