Mechanical ventilation under GA causes changes in lung & cardiovascular physiology. The function of the respiratory system is further affected by abdominal surgery. Obesity contributes to further pathophysiological changes in respiratory system making these patients more prone to perioperative complications, such as hypoxemia, hypercapnia & atelectasis. Impaired oxygenation of blood occurs in most subjects who are anesthetized. Immediately after the induction of GA, atelectasis develops, leading to a reduction in both V/Q ratio & pulmonary compliance, even in non-obese patients and is thought to be responsible for perioperative respiratory complications, contributing to additional morbidity & healthcare costs.
