Objective: To study the efficiency of vaginal progesterone alone versus vaginal progesterone in addition to cervical cerclage in patients with short cervix for prevention of preterm labor. Design: Randomized clinical trial Setting: Department of Obstetrics and Gynecology, Cairo University Study Period: June 2010 to May 2014 Participants: After screening of 142 pregnant women visiting the outpatient clinic with history of one or more preterm deliveries patients, 60 women with a cervical length < 25 mm were recruited in the study. All women were between 11 and 15 weeks of gestation with a singleton fetus. Methods: Participants were randomized into two groups. Group I (n=30) received daily natural progesterone 100 mg capsules up to the end of 34 weeks in addition to McDonald{u2019}s Cerclage operation. Group II (n=30) received natural progesterone capsules the same way as in group I. All women were followed up until termination of pregnancy. Uterine contractions were monitored by an external tocodynamometer. When the test is positive patients received corticosteroids enhance fetal lung maturity till the gestational age of 34 weeks. Upon inevitable preterm delivery, the cerclage suture was removed (for group I) and the patient was delivered. The cerclage suture was removed after completing 37 weeks of gestation for all patients in group I. The primary end point of the study was achieving 34 weeks of gestation and the secondary end point was achieving 37 weeks of gestation
